Filing for AOS - What's the deal with the medical forms?

Hey all,

I've got a little bit of time before I actually need to deal with this, but as I understand it, the form they gave me at the medical is not sufficient for AOS. All I ave is the vaccination supplement. I presumed everything else was in the mysterious brown envelope.

So, how am I meant to take a form to a civil surgeon to be transcribed? I am confused as hell!

I have a stamped headed receipt that states I had the medical....

What was everyone else given when they went for the medical and what did you do for AOS?

I just mailed the DS???? that I got at UK medical. I know many have had success doing this. Worst case is that I get RFE.

I got lucky, they didn't ask at the Adjustment of Status interview. All we supplied was the Health Dept form showing additional vacinations and the supplement form from the Embassy or Medical.

That's not the norm, my brother got hit up for the I-693 form. They want the form filled out and signed by a doctor and then placed in a sealed envelope, by the doctor. A pain, but a local doctor looked at records and filled it out for him at no charge. I doubt that's the norm either!

Are all the required vaccinations current? If so everything should be shown on the Supplement. The person doing the interview probably isn't qualified to evaluate vaccinations, hence the docter review. Just a guess. There's an online list of approved doctors on the USCIS web site.

All my vaccinations are up to date, but I can't show my blood test results etc. I just don't have that stuff........they never gave it to me.

I sent the form I received at the K1 medical, and I was approved without interview or RFE.

My daughters case however, received a RFE, so I then went ahead and transferred the info. Cost about $350.

My advice would be to send what you have and keep your fingers crossed.

Basically all you need to do is fill in part 1, part 2 number 5 and part 5 of the I-693. Vaccinations is the focus.

When was your medical? Here's a link to the new info in regards to exams... Great link that explains this stuff pretty clearly... what's necessary in terms of vaccines and TB test requirement since the form has changed.
