What name do I use on forms?

I've looked through the FAQ's, and past forums about this and couldn't really find a definite answer..

I am still confused about what name to use on all my forms for adjustment of status.

I will be getting married in October this year (changing status from an F-1 student visa) and my middle name is going to be my maiden name, last name is going to be my married name.

So will I use "[first name] [maiden name] [husband's last name]" on the application and other forms EVEN THOUGH my health documents, birth certificate and other supporting documents have my maiden name on them?

Or do I use my maiden name just like the medical exam/and other documents say?

How will USCIS see that I am that same person if I have different names on various forms...?

Sorry but I just want to make sure!

You will use your marrried name on the forms and you will use your maiden name where it is requested. Most of the forms ask for both names, some say other names used, some ask for maiden names. When you read the forms it should be easier to figure out.

thank you for the reply! i'll look over the USCIS forms..

but the confusing thing for me is, i'm going to get my medical exam done before i get married...

and my name on the driver's license is still of course my maiden name, so will I just write my maden name on the medical exam form?

If you list your other names on the form they will figure it out....

Since you are not yet married, use your maiden name. After marriage, your married name. You'll do fine, try to relax a bit.
