The visa for my spouse was approved (10 months) by the Tokyo Embassy, but she gave birth two months after that (in Japan).

Unfortunately, the baby had some medical problems and was in the NICU for 40 days. It now looks like we cannot enter the US within the six-month period.

Would we be able to apply for an extension of that six month period to enter the US?

How is that done?

That is correct. The OP can simply check the expiration date on the visa to confirm.

OK, I think that I just answered my own question. I found this info on the Australian Embassy web site. (This info was not on the Japanese Embassy web site, unfortunately.)

What if I don’t use the visa within the six months? Can it be extended?

Immigrant and fiancé(e) visas cannot be extended. If the visa is not used within its period of validity, you must return it to this office for cancellation, along with a note explaining the reasons why the visa was not used. Upon return of the visa and explanation, we will inform you as to the requirements needed to have new visas issued.

Anyone have anything to add?

The answer you posted above is correct for IR(CR)-1 and K-1 visas.. NOT K-3

The answer you posted above is correct for IR(CR)-1 and K-1 visas.. NOT K-3

I suspect the answer is similar for K3 with the question being, "what if....two years...?"

Your wife has two years to use the K3 visa.
