On the I-751 application part 5 it ask for information on your children, does this mean children born to your marriage or could it include my children from my ex husband? I am not sure if they mean all children in the family whether or not they were born in the marriage because all of my children are US citizens like I am. Also, on part 4 it ask for your family name, does this mean the name that I have now with my husband or my name I had with my ex husband or my maiden name before I ever got marrried? Sorry for all of the questions but I want to make sure I have it right. Thanks to everyone for helping me out:))

I had the same question and never really got an answer. So, in view of lack to clear instructions to the contrary I just worked on the premise that step-children who were US citizens were not included and so I did not include my husband's daughter on the form. If they were my children from a previous relationship, I would, however, include them as they may have future immigration implications. For family name I also assumed they mean the one you are currently using as that made the most sense, so if that is your new married name write that; if it is your pre-married name, write that; if it is your maiden name, write that. Include the other names where it lists 'Any other names used".

The form is primarily to be filled out from your foreign born spouse's perspective as he/she is the one adjusting status so approach the form from that viewpoint. They are not requesting the information about the US citizen so don't include your information if you are the US citizen - use your spouse's. If you have changed your name because of the marriage, however, the family name information they request is still valid where it asks about the US spouse.
