Do we need to fill out forms for SSN ? If so, can we get the forms online ? Thanks

What is your immigration status and no you do not need to file out an application before going into an SSA office. The SSA employee will enter all your info into the system and then printout an application for you to signed after you have reviewed that all the info is correct.

When the applicant remains in the office throughout the interview and keying of this data, the FO employee must print the systems-generated application for the applicant's review and signature, and the FO employee(s) certification(s).

I just printed the form online and wrote on it using Photoshop. The signature has to be original though.

I just printed the form online and wrote on it using Photoshop. The signature has to be original though.

And it will be tossed in the trash. Once again you don't need the SS-5 if you are applying for an original SSN or replacement card and you are applying at the office.

This is SSA procedure, not something I dreamed up.

Oh well, I got my SSN card a week after that. It is my first card.

Printing Forms

SSA offers our forms in Portable Document Format (PDF). PDF files are printer independent using any printer. To read and print a PDF publication, you must have the Adobe Acrobat Reader software installed on your PC. You can download the Adobe Acrobat Reader version suitable for your system by clicking on this button. There is no charge for this software.

Get Acrobat Reader

SSA forms must be printed on 8 1/2" x 11" white paper. You must print the form with blue or black ink to be acceptable to SSA.

NOTE: if you live abroad, and cannot obtain 8.5 x 11 paper, the use of A4 size paper (8.25 x 11.7) is the only acceptable alternative.

3. Systems-Generated Application

request the applicant to sign the systems-generated application (even if the applicant had brought in a completed paper or Internet Form SS-5);

keep the signed systems-generated application in the holding file for later shipment to the SSA Records Center; and

destroy (i.e., by shredding) any other versions of the form that may have been completed once the systems-generated application has been signed.
