Greetings VJ!

Quick Quesitons Please :huh:

1. Can I use the same notorized bank letter which states my balance that I used this past June for my husband's Visa interview to file for the I-864 Affidavit Of Support this month AUgust? (It will only be a two months laps...from June to August because I will be sending in the application this Month) or do I need a new updated bank letter?.

2. Do I need to send in Bank statements if I am sending in a bank letter? When my husband went for his Visa interview the consulate only collected the bank letter and didnt even look at the bank statements

3. Do I need to add my birth certificate also? ( I know my husband definately has to add his)

4. Which last name do I use to sign the I-864 Affidavit Of Support form? (We are now married and I have his last name but I have only changed my bank account to his last name and my license...everything including my paystubs still have my maiden last name)

These are the documents that I will be sending please let me know if I am missing anything.

FORMS: I-485 AOS application, G-325A Biographic, I-765 EAD and I-864 Affidavit of support. The supporting documents are: Taxes 2007-2005, Birth Cert husband, I-94, Visa stamp, Passport pic and pages of entry, Social Security Card, NOA2, Paystubs from me, Bank letter from me from June?, and that is it.. am I missing anything? :help:

TIME LINE

09/2007 - Gathered all documents for I-129F petition.

11/09/2007 - Sent I-129F

11/21/2007 - Package sent back to me( I forgot to sign one of my G-325A forms) I cried my eyes out but sent it next day delivery back to USCIS.

11/27/2007 - Received NOA1

02/13/2008 - Received NOA2

02/20/2008 - Package left NVC

02/22/2008 - Case uploaded into Ghana consulate data system

04/08/2008 - Packet 3 & 4 sent to fiance

04/16/2008 - Packet 3 & 4 received by fiance

06/24/2008 - Interview date (God blessings on this day)

06/24/2008 - DENIED visa at interview

06/25/2008 - APPROVED visa at 2nd interview with senior consulate

06/27/2008 - Picked up VISA

07/03/2008 - Fiance entered USA at JFK POE & Got EAD Stamp in passport

07/18/2008 - Married

08/07/2008 - Mailed AOS application

Bank letter should be fine

SSN card not required

unless self employed or other specific reason only 2007 tax return is needed

your bc not needed

Hi! In regards to your last name... You're the sponsor, right? I had the same question and got conflicting answers. Here's a link to my thread.

Forms: Are you going to do the AP I-131? Did you included the I-693? You don't need to include the Social Security card. Marriage certificate. Photos.

Also, don't forget to include supporting documents for your I-765 (and if you decide to do the AP I-131, you will also need to include supporting documents for that as well).

Be sure to organize your packet well and include a cover letter.

AOS & getting the SSN & the I-693

6/13/08 Applied for SSN for David ----- TRIP 1; 6/23/08 Went back to SSA 'cuz the person didn't pc all documents (rec'd letter from them) ----- TRIP 2

7/2/08 Received David's health insurance cards! Yay!

7/3/08 Went back to SSA; they said come back 7/9 if he still hasn't received it ----- TRIP 3

7/8/08 Got list of vaccines he still needed from CS by fax/phone; tested for immunity to MMR (only vaccine still needed) at our family dr

7/9 & 7/10/08 Went back to SSA, card still not received - Denied opportunity to apply for replacement ----- TRIP 4; Went back to SSA again to apply for a replacement card - Talked to a supervisor this time, POE people entered his last name incorrectly ----- TRIP 5; went to get David's ID thru the State, denied - instructions wrong on website; measles test inconclusive, another test needed; requested new lease

7/14/08 SSA called me in regards to problems at local SSA office

7/15/08 INFOPASS appt to fix POE name input error and then went to airport to fix POE name error

7/17/08 Received SSN card in mail and went to get state ID

7/21/08 State ID received in the mail and tested again for measles immunity

7/29/08 MMR shot #2 given and dropped off papers at Civil Surgeon (CS)

7/31/08 Picked up I-693, completed by local CS, (I paid $25, don't be fooled)

8/3 - 8/6/08 Worked on forms I-485, G-325A, I-864, I-765 and I-131; sent to Chicago - YESSS!

8/11/08 NOA1 for AOS, EAD and AP; 9/2/08 Transfered to CSC; 9/4/08 Biometrics

10/24/08 Rec'd AP; 10/27/08 Rec'd EAD; 11/24/08 Rec'd GC

The form I-693 is it the vaccination report? Because that is what I will be sending.

The supporting evidence is: 2 passport photos, I-94 copy, visa and passport copies.

Yep, the I-693 is basically the vaccination report for a K-1 visa holder (but it really is a medical exam form). You may have a DS-3025.
