Hi guys,

I am due to fly out to the States on Monday (so much stuff to do before then!!) and Nikki and I want to get married and get the AOS under way ASAP as we want to apply for AP to come back here for Christmas. I have called a few Civil Surgeons in the North Mississippi & Memphis area and none seem to have any idea about transferring the information from my DS-3025 to the I-693, all want to book me in for another medical and seeing as I already forked out for my medical over here only 3 months ago, I am obviously reluctant to pay again. I know a lot of people used Dr. Arnold on here, is he a good route to go down? I've noticed on the I-693 instructions that a Civil Surgeon should be present to transcribe the information, as Dr. Arnold is in Texas, has anyone had any problems with this (ie. the USCIS asking why the form was completed in a different state). How long does it take Dr. Arnold to complete the form and send it back? As I said we are trying to get the forms sent off in the next couple of weeks so time is of the essence.

Regarding the Affidavit, I am going to ask my Uncle to sponsor me as he sposored me for the K-1 and Nikki has only just finished college and will not qualify to sponsor me. Could I use the same letters, from my Uncles employer and bank, that I used for the K-1 Affidavit for the I-864? They are both dated in the last week of April, is this too far in the past?

If it were me I would just send the DS-3025 and not worry about the transcription....

But if you want to know how fast Dr Arnold is call her. You should anyway to give her office a heads up as to what is coming. The USCIS is a government agency. As long as it is prepared by an approved CS they could care less where from.

Also keep in mind that you will need a new set of shots that the USCIS is requesting for AOS. They are based on age and gender and you may have had them already, but if you haven't, you will most likely have to get them again.

Hi...

Great link that explains this stuff pretty clearly... what's necessary in terms of vaccines and TB test requirement since the form has changed (all new requirements re: the test and vaccines went into effect 8/1/08!).

Here are the most impt parts from the link itself:

Q: If an applicant is required to receive only the vaccinations, and not undergo the entire medical exam, does he/she have to submit the entire Form I-693?

No. Applicants who are not required to have the entire medical examination need to submit only pages one, three and five of the new form. The required sections include Part 1, Information About You, the vaccination record portion of Part 2, and Part 5 (if the vaccinations are administered by a civil surgeon) or Part 6 (if the vaccinations are administered by a local health department). Pages two and four, that would have been left blank, do not need to be submitted.

Q. I had my medical exam completed before the new TB or vaccination requirements went into effect, and have not yet submitted it to USCIS, or I have submitted it but it has not yet been reviewed. Am I now required to have a new medical exam that meets the new TB and/or vaccination requirements before I can adjust status?

Generally, no. When reviewing a Form I-693, USCIS will note the date the exam was conducted and refer to the Technical Instructions, medical requirements, and form edition that were in effect at that particular time, remembering that for each revision of the Technical Instructions there was a 30-day grace period. If the civil surgeon conducted the medical exam properly and in keeping with the exam requirements and form edition in effect on that particular date, there is no need to return for a second exam or to have the results recorded on a newer edition of Form I-693, as long as you submit the Form I-693 before its one-year expiration.

7/8/08 Got list of vaccines he still needed from CS by fax/phone; tested for immunity to MMR (only vaccine still needed) at our family dr

7/9 & 7/10/08 Went back to SSA, card still not received - Denied opportunity to apply for replacement ----- TRIP 4; Went back to SSA again to apply for a replacement card - Talked to a supervisor this time, POE people entered his last name incorrectly ----- TRIP 5; went to get David's ID thru the State, denied - instructions wrong on website; measles test inconclusive, another test needed; requested new lease

Beware of using doctors from another state that you haven't even seen in person. I read someone's experience that she used Dr Arnold and was asked about it at the AOS interview. It would seem inexpensive than other civil surgeons, but at the same time very risky. I don't know how it affects that person's AOS process, but since it was brought up, I'm not sure it would be best for everyone.

I read a post by another member where he said that they were questioned about why they used Dr. Arnold. It all boiled down to why he signed it when he had not seen the person since they lived in another state. Bottom line is, he was just transcribing the info the medical doctor in their home country had written regarding the vaccines needed for AOS. Their AOS was approved since it's legit to have any surgeon do so, especially if you had your medical done before the interview.

I have just been looking at the DS-3025 copy, that I was given by the Physician over here, and at the I-693. Firstly, I only seem to have been given page 1 of 2 for the DS-3025 (just the vaccination docs) was this the same for everyone else or did they get both pages? I just want to make sure I have everything I need.

I do not think I need any further vaccinations or TB tests as I had the TB X-Ray for my medical over here and my physician has marked on my DS-3025 that I am not age appropriate for Rotavirus, Hepatitis A, Meningococcal, Human papillomavirus and Zos.

Anyone with any advice on my I-864 query as I will be contacting my Uncle later.

Hi, I also used the same letters. :) I actually had an INFOPASS appt (about something else) and the woman I spoke with there specifically told me that this was fine as I had just done the I-134 within the last few months (also from April). What I did add, however, was newer copies of checks from work.

To add to this post, keep in mind you only need to start taking the new vaccines if age appropriate, but do not need to complete them. For instance, both TB and HPV require multiple shots over several months, but the series does not need to be completed by the time of AOS.
