Medical, finally done!

It's good to be back home now.

I arrived Manila from Cagayan de Oro July 27. July 28 was my first day of my medical at St. Luke Extention Clinic.

What to Bring: For Guard Check up

1. Passport

2. Appointment Letter from US Embassy ( on my case i brought with me, the email confirmation for my interview)

After the guard check your passport and appointment letter from Embassy, he will give you a form for you to fill up with a priority # for picture taking.

Note: You have to bring 1 pc 2x2 ID picture on your first day, and 2 pcs 2x2 ID picture on the 2nd day. (on your 2x2 ID picture, make sure that your ears are visible and no earrings. ) You have to put your signature over printer name at the back of the 2x2 ID picture.

After you fill up the form, see whether your priority # is called, if not, then wait. If your # is called already and you failed to notice it, just go direct to the picture taking room.

Note: Don't be in a hurry in filing up the forms for the reason that you're afraid if your # will be called, you need to answer it correcty. Slowly but surely. In case you're # is already called, then right after you're done with the form, you can just go direct to the picture taking room.

After picture taking, you have to take back with you those forms that the photographer took. Outside the picture taking room, you will see applicants who form a line in a counter, you have to do the same, stand on the last applicant who fall in line. When it's your turn already, SLEC staff will get those form that you just filled-up, they will check if you answer it correctly. Then, you will be advised to wait again til your name is called, then thats the time that you have to go to 5th floor for payment.

Note: When you signed forms or any form of document at SLEC, make sure that your signature is the same as what have been reflected on your passport.

After paying the medical fee, you will be advised by the cashier to go to X-ray floor (i forgot what foor was that, heheh), it may not be on the X-ray always at first, just follow what the cashier told you. Then, from X-ray to Laboratory floor for Urine and blood test. After that you will be advised by the laboratory counter staff to go to US Counter (i forgot again what floor was that, hehehe) for your height, weight, blood pressure, pulse rate and eye test. After that, the usual is the US counter will give you a priority # for your Doctor's assignment for your physical exam where you are ask to take off all your clothes. Since I have my period on that day, so i didnt do that yet on my first day, instead i was advised to go to the floor where immunization interview take place.

I went there, and i was interviewed by a very nice doctor about my immunization record. These are the questions asked on me:

1. What is your name?

2. Who is your petitioner?

3. When is your first and last day of menstration?

4. Did you ever attempt to do suicide in the past?

5. Did you ever have chicken fox before?

5. Where are the marks?

6. Do you have previous records of your X-Ray?

7. Are there anyone in the family that has TB?

8. Where you expose to an environment with TB patients?

9. Are you allergic with some medicines?

10. Do you have problem in breathing?

11. Where you immunized before? Are there any marks?

12. How old I am?

13. Did i undergone operation before?

14. Did i get hospitalized for a long time? if yes, what was the cause?

After immunization, i was told my the doctor to go to the CHECK-OUT COUNTER and just be back when my period is over. Upon check-out, they gave me the Reciept with a little piece of ond paper stating TCB (To Come Back) after menstration.

July 31, End of my mens, so i went back to SLEC to continue my medical. I just presented to the guard the reciept and that small piece of bond paper stating that i am advised to come back after my mens. I went to the information section located at the entrance, and they told me to go to the US COUNTER Floor. So i went up to that floor and presented to them the receipt and that small piece of a bond paper with a note. Then, the US COUNTER In-charge gave me a priority # with my doctor assignment, i was assigned to Dr. MGTT (something like that, cant recall). When it was my turn, i went inside the Doctor's room, and greeted her with a warm morning greetings. She smiled and she told me to sit down. I was ask a couple of questions like:

1. What's your name?

2. Who is your petitioner?

3. First and last day or menstration

4. Did i even commited or attempted suicide before?

5. Do i have allergies with some medicines?

6. How old i am?

7. Did i undergone operation before?

Then she told me to take off all my clothes (as in naked), then ask me to wear the dress provided by the hospital. I lay down on the bed and the doctor, tried to check my neck, my wrist (pulse), my breast, lower part of my stomach, my back, butt, etc etc (general physical check up in short). After that, of course she told me to get dressed and she let me signed a paper, then i went back to US counter. US COUNTER then, advised me to go to Immunization room for immunization. It took a while til my name was called.

I was given, 3 vaccines, it was TDAP, MMR and Anti-Cervical Cancer vaccines. The latter was very painful, but it's alright. After that, i was told to go to the RELEASING COUNTER, observe the effect of the vaccines in 30 minutes.

Finally after about 30-40 minutes, my name was called for releasing. I was ask to provide them 2 psc of 2x2 ID picture (with signature over printed name at the back). Waited a couple of minutes, and again my name was called. The In-Charge handed to me the X-ray film and the vaccination record, and told me to bring that when i go to US. They keep my passport together with my medical result. They told me that they will be the one who will forward it to USEM.

That was the end of my medical. Thank God.

i have a question regarding the picture..is it really necessary to exposed the ears? coz i already have pic but the right ear didnt clearly exposed..

Thanks lyka.... Yeah, it is necessary that you have to show the ears without any earings. I have noticed some that are asked to take a photo again because their ears are covered with their hair. Good luck to you.

Thanks lyka.... Yeah, it is necessary that you have to show the ears without any earings. I have noticed some that are asked to take a photo again because their ears are covered with their hair. Good luck to you.

Nov 15 - Our Wedding Day!
