I just got confused while filling up Q#9 on DS156-"List All Countries You have entered in the last 10 years" - does it apply only to those who stayed there for a period of 6 mos or more?I've visited other countries with my son before but only for more than a week,are they counted?

Also,i have a 12yr old son that will go with me(K2) do i also have to fill up for him the DS157?

Question 9: Yes u have to put countries where u been even its only a week.

Ur son dont need DS-157,Its only you who need to fill up DS-157 form.To make sure....u can PM dbears bcoz she's done with her interview with her 2 kids (K2).

Question 9: Yes u have to put countries where u been even its only a week.

Ur son dont need DS-157,Its only you who need to fill up DS-157 form.To make sure....u can PM dbears bcoz she's done with her interview with her 2 kids (K2).

do not believe that is correct. the child does need to submit a DS-157. it's the DS-156k that does not need to be submitted for the child.

my daughter came here on K-2.

2 DS 157 needs to be filed out for kids also. Pay at bank naman. NO free rides!

do not believe that is correct. the child does need to submit a DS-157. it's the DS-156k that does not need to be submitted for the child.

my daughter came here on K-2.

Oh,ok.i bet you're right coz when i checked again the DS156K it's all directed to the fiancee.

I just went through the K1/K2 process myself.

the forms for K2 would be:

2 copies of DS 157 (1 pre-filled online + 1 blank form from the bank --after paying visa fee-- which you can fill up handwritten, preferably using black pen)

2 copies of DS 156

his NSO certified birth certificate

and pls don't forget to bring pictures of you and your son when he was still baby, during his toddler years, preschool years. etc. basta growing up pictures na kasama ka.. di ko alam reason but it was asked from me by the consul (same thing with another applicant who have kids). It was the only thing that i was not really ready, buti nalang may print out ako ng photoblog ko for my kids, kahit konti may naipakita ako.

:help: with Question#8 on DS156 - "Other Surnames Used (Maiden, Religious, Professional, Aliases)"

I had my 2 old passports with my incorrect surname written on it,when i first applied for it,the DFA staff told me to just use what's written on my BC which at that time i havent had it corrected yet(a 1 letter error on my surname),should include it on my answer on Q#8?

also,my full name is completely different from my nickname and it's the name(nickname) i've been using on our chatlogs and emails,should i likewise include it on Q#8?
