Hello all,

I am currently trying to get all the paperwork for my AOS together. I filled out all forms except the I-693 Medical Examination of Aliens Seeking Adjustment of Status. I had a medical examination in April for my K1 visa. So according to the information in the guide, I only need to submit a vaccination supplement (I-693A), not the entire medical report. The thing is, that I don't have this. Any paperwork that the doctor filled out during my medical, was sent directly from the doctor's office to the US consulate. So I never saw any of it. It probably ended up in my file in the brown envelope that was taken by the customs officer at JFK when I arrived here. So I never saw it and I don't have it. All I have is my vaccination booklet and I'm guessing that a copy of that isn't enough. What do I do now? Is there any way I could get that I-693A filled out by a physician here if I show my vaccination booklet? That would solve the problem. Or is there another way around this? Why do these things have to be so complicated?? :huh:

Great link that explains this stuff pretty clearly... what's necessary in terms of vaccines and TB test requirement since the form has changed (all new requirements re: the test and vaccines went into effect 8/1/08!).

Here are the most impt parts from the link itself:

Q: If an applicant is required to receive only the vaccinations, and not undergo the entire medical exam, does he/she have to submit the entire Form I-693?

No. Applicants who are not required to have the entire medical examination need to submit only pages one, three and five of the new form. The required sections include Part 1, Information About You, the vaccination record portion of Part 2, and Part 5 (if the vaccinations are administered by a civil surgeon) or Part 6 (if the vaccinations are administered by a local health department). Pages two and four, that would have been left blank, do not need to be submitted.

Q. I had my medical exam completed before the new TB or vaccination requirements went into effect, and have not yet submitted it to USCIS, or I have submitted it but it has not yet been reviewed. Am I now required to have a new medical exam that meets the new TB and/or vaccination requirements before I can adjust status?

Generally, no. When reviewing a Form I-693, USCIS will note the date the exam was conducted and refer to the Technical Instructions, medical requirements, and form edition that were in effect at that particular time, remembering that for each revision of the Technical Instructions there was a 30-day grace period. If the civil surgeon conducted the medical exam properly and in keeping with the exam requirements and form edition in effect on that particular date, there is no need to return for a second exam or to have the results recorded on a newer edition of Form I-693, as long as you submit the Form I-693 before its one-year expiration.

Be sure to follow the directions, as you only need to complete a small portion of the paper; the same goes for the doctor (the Civil Surgeon) Explain to the CS that you only need your vaccines transcribed onto the I-693. You shouldn't pay much for this. I paid $25. Call more than one Civil Surgeon to be sure to get the best deal.

AOS & getting the SSN & the I-693

6/13/08 Applied for SSN for David ----- TRIP 1; 6/23/08 Went back to SSA 'cuz the person didn't pc all documents (rec'd letter from them) ----- TRIP 2

7/2/08 Received David's health insurance cards! Yay!

7/3/08 Went back to SSA; they said come back 7/9 if he still hasn't received it ----- TRIP 3

7/8/08 Got list of vaccines he still needed from CS by fax/phone; tested for immunity to MMR (only vaccine still needed) at our family dr

7/9 & 7/10/08 Went back to SSA, card still not received - Denied opportunity to apply for replacement ----- TRIP 4; Went back to SSA again to apply for a replacement card - Talked to a supervisor this time, POE people entered his last name incorrectly ----- TRIP 5; went to get David's ID thru the State, denied - instructions wrong on website; measles test inconclusive, another test needed; requested new lease

7/14/08 SSA called me in regards to problems at local SSA office

7/15/08 INFOPASS appt to fix POE name input error and then went to airport to fix POE name error

7/17/08 Received SSN card in mail and went to get state ID

7/21/08 State ID received in the mail and tested again for measles immunity

7/29/08 MMR shot #2 given and dropped off papers at Civil Surgeon (CS)

7/31/08 Picked up I-693, completed by local CS, (I paid $25, don't be fooled)

8/3 - 8/6/08 Worked on forms I-485, G-325A, I-864, I-765 and I-131; sent to Chicago - YESSS!

8/11/08 NOA1 for AOS, EAD and AP; 9/2/08 Transfered to CSC; 9/4/08 Biometrics

10/24/08 Rec'd AP; 10/27/08 Rec'd EAD; 11/24/08 Rec'd GC

My first post...... and I am not the expert, but I just filed my 693 as well. The doctor (in FL) filled out part 2 (vaccination portion) put it in a sealed envelope (signed/stamped of course) and I mailed it with my package. So far so good. I just received my biometrics appointment and heard nothing from USCIS requiring me to reaccomplish any paperwork. Hope this helps.

Hi, I face the same problem. In addition, I am a vaccine exempt K-1 Visa Holder but I have no proof with me right now. I underwent a medical exam in Hong Kong without taking any vaccine to qualify the K-1 Visa.

Now, I have no proof to submit with my AOS that I am vaccine exempt. What should I do?

I have the following questions:

1) Is it still necessary to fill out I-693?

2) If yes, is it necessary to ask a local civil surgeon to fill out his parts?

3) What is I-693A? I cannot find this form online?

Before I call a local civil surgeon and the clinic I consulted in Hong Kong, can anyone give me some clues? I am so confused.
