Does all the members in the family have to have the medical examination?

YES they are immigrating to the USA, and are being checked for medical conditions that would make them in-admissable.

Also: appropriate vaccinations will be needed of all family members that are immigrating on an IR or CR visa.

Note: There is a website that specializes in immigrations through Guangzhou counselate, that board has info on preparing for interview, where to get medicals done, etc... "A Candle for Love" http://candleforlove.com

One note: If you are doing CR-1 then any dependent children accompanying spouse will need their own I-130 filed for their own visas, unless the child is a US citizen, your biological child.

Children immigrating with a spouse on CR-1 or IR-1 do automatically a visa like K-3/K-4, there is no derivative CR/IR-Visa for step children.
