Hi guys, I am now organizing my papers for filing the Adjustment of Status application. I understand I should make a cover LETTER and I've found some samples here in VJ, can anyone show me a sample of the overall cover PAGE though? Or is it just the same with the cover letter?

Also, does the Birth certificate which needs to be submitted along with the other documents have to be an original one? or a photocopy will do?

photocopies are fine.

Honestly, you don't need two cover letters. One will suffice.

Oh, by the way, did you compile your forms in a folder with a slide at the side, or just an envelope with some paper clips on it? My husband said I need not to make it fancy because they are going to sort out the papers anyway.

I put together each form with their required documents with a binder clip and put them in individual envelopes and labeled them.

They do rearrange the paperwork in the order they want. I know this because when I submitted my AOS I forgot to sign one of the cheques. They sent back my entire package, and I briefly flipped through the stack of papers -- they were definitely not in the order I had submitted them. But that's just me.

Hi! We are on the same situation right now about preparing all the docs to file for my AOS also, BUT i have a question i hope you could tell me..have you undergone that TB SKIN TEST as the NEW REQUIREMENTS NOW? OR WHAT did you do with your missing vaccinations?

I havent had my vaccinations yet, actually. The I-693A form is the only thing I dont have yet, coz my vaccinations are scheduled this Aug. 11th. No I have not undergone the TB skin test. Is that really required even for those who have clear chest X-rays?

Form I-693A is NOT valid any longer. As of May 2008 (I think), the correct form to use is I-693. There is a vaccinations part in there. Read the instructions on that form so you know which sections need to be completed.

Yeah, I have just read USCIS's update on the new TB testing and the vaccinations. Looks like I will have to undergo that TST. Well, I havent gone to the civil surgeon yet for my vaccinations, so we'll see what she says.
