A full medical can cost $200 - $400 depending on doctor.

They are more than likely requesting the vaccinations record part if you had medical for your K-Visa less than a year ago.

I had to visit the doctor with I-485 instructions in hand and point at the section about K-Visas, he quickly reduced fee from a few hundred down to $35.

A full medical is NOT a requirement for AOS from a K-Visa only the vaccination part of I-693 is.

Recently USCIS has updated form I-693 with updated directions for Civil Surgeons explicitly explaining that ALL K-Visas do not need a full physical.

I-693:

2. What if I am a K nonimmigrant visa holder and already had a medical exam overseas?

If you were admitted as a:

A. K-1 fiancé(e) or a K-2 child of a K-1 fiancé(e), or as a

B. K-3 spouse of a U.S. citizen or a K-4 child of a K-3 spouse of a U.S. citizen, and

C. You received a medical examination prior to admission, then-

a. You are not required to have another medical examination as long as your Form I-485, Application to Register Permanent Residence or Adjust Status, is filed within one year of your overseas medical examination.

b. You will, however, be required to complete Part 1, Information about you, and submit the vaccination section of Part 2 with your adjustment of status application. A designated civil surgeon must complete the vaccination section and Part 5, the Civil Surgeon's Certification.

And form I-485 directions have this:

8. Fiancé(e)s.

If you are a K-1 fiancé(e) or K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiancé(e) visa, you only need to submit a vaccination supplement, not the entire medical report. You may include the vaccination supplement with your adjustment-of-status application.

Bring vaccination record, or get immigrations related vaccinations and get the record transfered to the I-693 by a US civil surgeon.

Hmm can you specify further you status and your doing AOS based from? And what area are you in? Here is a link wherein you can find Civil surgeons near your area where you can have your Medical exam done https://egov.uscis.gov/crisgwi/go?action=of...office_type=CIV just type in your zip code. The medical exam cost varied from civil surgeon to civil surgeon. Sometimes if your luck you will find one who will charge you a reasonable price. Here is an updated requirements for Tb and vaccination http://www.uscis.gov/files/article/FAQ_TB_...ents_072408.pdf. Hope this helps.

Looks like they are adjusting from a K-3 (The approved I-130), you may want to look at their timeline.

It's impossible to tell whether they had their medical within the past year. There are many dates missing in there. They got the NOA2 in Aug 2006 and interviewed in Oct 2007. One can safely assume they had the medical between the NOA2 and the interview, and probably closer to the interview than the NOA2.
