I got an RFE for our I-693 based on the one we submitted "Page 3, Part 2, Section 5 is blank or missing". I called the doctor that we used and the receptionist said the I-693 was complete. When I asked for a copy, she said they weren't allowed to give me a copy. They can only give us a copy of the immunization record. I can't believe that a patient isn't allowed a copy of their medical records and that includes the I-693.

Does anyone know anything about this issue (before I get to the doctor's office and go ballistic!)

You need to bring the RFE to the doctor's office to show them and they SHOULD respond to it if they won't give you the paperwork to do it yourself.

But you're right, it sounds like a bunch of ####### that you can't see your own medical records. :wacko:

While I agree with kid brooklyn, you're going to have to get a copy of what they submitted or if they refuse, then take a copy of the "immunization record", and mail that back, advising that the Civil Surgeon won't give you another copy of the letter. I know when I got mine from the Dr. , they placed it in a sealed envelope and I sent that away. The office told me that if it was lost or they needed another one, to just call them, and they'd give me a copy.

I know in the past my husband has had issue about Dr.'s not giving out his own medical information to him, and he said it was his right...he found out, that it is...at least with that Dr. he saw...at a cost. Highway robbery!

4/29/13 - Case Complete!

5/10/13 - Interview notice emailed.

Instructions for Sivil Surgeon:

.... Return all supporting medical documents to the applicant

and give them a copy of the vaccination record. Return all

supporting medical documents, including chest X-rays (if

obtained), directly to the applicant. In addition, give the

applicant a copy of the completed vaccination record in

Part 2. This will serve as the applicant's official vaccination

record and may be retained by the applicant for future use in

establishing compliance with vaccination

requirements. (Example: school, day care, employment, etc.)...

My report was sent by mail within a week, but I didn't receive a copy for my records. I called the clinic, and they mailed me the whole thing, though here clearly sattes only vaccination record. Be persistant.

Hi sorry that happened to you. THe civil surgeon gave us the sealed envelope but at the same gave us COPY OF EXACTLY WHAT IS INSIDE THE SEALED ENVELOPE. So they should give you a copy too especially the PART2 section 5 because that is the supplemental form for immunization. Good luck hope everything went well.

Well, we returned to the doc's today. I was shown what they sent and it turns out they must have used an old form because there was no page 5. They had her immunizations on a page titled "Supplemental Form to I-693". It was, in fact, the same information but not a "Page 5". I asked them to re-do it and they were glad to take care of it. Then I had to ask them to put it in a sealed envelope and finally I had the doctor sign over the tape. I hope that satisfies the immigration folks. The doctor's office, again, refused to give me a copy of the 693 (they did give me a copy of the immunization page) but since they were so nice about re-doing everything, I didn't force the issue. If I get any more flack from the immigration folks, I guess I'll have to make a fuss with the doc.

Thanks for your comments. Since some of you did get a copy of the 693, it's obviously not against any law.
