My wife came with no vaccination records. We are now filing AOS/EAD and I am aware that she will need to have a vaccination suppliment to be completed by the civil surgeon. Our civil surgeon does not give shots so we must go to the health department. Will we be able to take the vaccination suppliment form to the health department and they will be able to give her the proper immunizations. My understanding is that after she is given the shots then she must return for the civil surgeon to sign the vaccination suppliment. Am I correct? What have others done when their new spouse did not bring immunzation records?Vaccinations/immunizations are not part of the k-1 medical work-up?

Vaccinations ARE part of the K1 medical. My husband got all his vaccinations before his interview, but they never gave him the worksheet. So he came without any proof.

From speaking with the civil surgeon, the only options without the worksheet are to

1. revaccinate

2. contact the physician overseas who conducted the K1 medical and try to get a copy of the worksheet

3. provide some other childhood record of immunizations.

My husband only got the MMR and the tetanus again. The others the civil surgeon marked as not age appropriate.

I imagine all civil surgeons will differ as to their policies.

Is the vaccination supplement required at time of AOS submission or is it needed at the time of interview? I'm wondering whether we will get an RFE if we submit our AOS application without it. My wife came on a K-1 visa, so I am assuming she does not require another medical exam.

You are correct that she does not need another medical exam, only the vaccination supplement.

I've heard of people who submitted their AOS paperwork with the worksheet received at the medical rather than the actual supplement signed by a civil surgeon, without getting an RFE.

There have also been people though who have been RFE'd for not submitting the actual supplement with the I-485.

Best be safe and submit it with your AOS paperwork.

We decided to get an I-693A (vaccination supplement) done at a USCIS civil surgeon since the I-485 instructions specifically state that K-1 visa applications should submit one --- didn't want to risk it.

We were lucky because James has all his vaccination records, and so it cost us (a relatively low, seeing other people's input on this) $50 for a doctor 25 mins away from us to fill out the 1-page document.

If you can find someone to do the same you may want to, I would just avoid any chance of an RFE since the process is already long enough from filing -> AOS interview/approval.

Good luck

From one Ukrainian household to another ....

Hope all goes well!

My wife brought her medical records (and English translation) with her so, in addition to the info that the clinic in Kiev completed, we also had back-up information.

Get the vacc supplement done by the C.S. and included it with your AOS filing. We didn't and got an RFE and then jumped through hoops to get it done! Plus it delayed EAD and AP too!

BTW...

shots "required" are:

2 varicella (chicken pox) given 30-days apart

2 MMR (Measels-Mumps-Rubella) given 30-days apart

1 Adult Tetanus shot (Td)

You may be able to exempt certain shots if the person has proof or can verify that they had certain diseases or if they have blood tithers which prove that they are immune to said diseases.

Hep B is no longer a requirement.

Hope this helps....
