My great grandparents came here from Germany and my last name is German, but I have never thought about saying I wanted to be called a German American

I agree with you to an extent.

I think (and I could be wrong) that when ppl say "african-american" or "whatever-american" they are giving homage to their heritage. But yes, the nationality in and of itself is based upon where you were born.

p.s. I would love for you to rewrite all of the demographic forms to reflect just that sentiment.
