HEY GUYS,JUST WANT TO KNOW HOW TO FILL UP DS 157... in questions.no.12 my questions are.... ive been working 3 years ago...after that i just work for my own my problem is did i really need to put my last 3 years of employment?i cant remember the names of my supervisors before....i dont know their telephone numbers...one of these company i worked before wasnt there on the address i worked before....what am i supposed to do?

hi... one of my employment records has no contact number,i hope its not a big deal,i just don't really know their contact number because when i was working there they don't have a phone,i just put NONE.

Just answer the best you can.

It can't be too difficult to find the address for a company? If they are not longer there then you can state that.

Don't overthink it. Just answer honestly and as thoroughly as you can.
