Firstly, it asks you to tick yes or no to the list of questions:

The question that asks if you are taking any medication, do you tick yes or no if you are on the pill (birth control) or is that not considered medication?

Secondly i had an operation to remove and ovarian cyst in december 2005, do i tick yes i have been hospitalised?? And if i tick yes, what exactly should i write to explain it? Do i attach a copy of the doctors report, would that be esaiest?

Thirdly, i don't have any immunisation records, will this be a problem, its a k-1 visa im applying for, so will i only have to have all the injections once i am in amceria and applying to AOS?

Biometrics - 5th October 2006 (22nd day)

RFE Sent Back - October 28th 2006

Transfered to CSC - November 15th 2006 (63rd day)

EAD Approved - December 11th 2006 (90th day)

AOS APPROVED!! - December 14th 2006 (93rd day)
