Hi,

I just called and left a voice mail for Dr Arnold Texas.

I also tried contacting few civil surgeons in my area..and everyone wants to charge atleast 150$ and up.

One of them said she HAS to do another complete physical on me when i kept telling her that.

Most of them are insisting on a physical..and I have been telling them that i already got that done just a month abck for

my k1 visa purpose.

Has anyone done all this in and around Chicago for a lesser and decent amount of money.

PLEASEEEEEEEEE let me know.That will save me some money and trauma!really! :)

I wish I could help you being in Chicago, but I needed the full exam done since I came here on a work visa so I had to pay $ 150.00 for mine.. :angry:

05/12/04 => Arrived in the US under L1 status / work visa

03/05/05 => Met the love of my life

07/02/07 => Married my wonderful USC husband

01/20/12 => Filed N-400 for Citizenship

06/21/12 => Citizenship interview: Approved!

07/25/12 => Oath Ceremony.. DONE WITH USCIS!!

What country did you come from to the USA? If it's the UK, there's a strong possibility you will be approved without filling an I-693 at all so long as you send in the vaccination supplement with your I-485 package that you should have been given at the medical.

I ran into the clueless receptionist when calling around for our I-693.

I simply brought a copy of I-485 directions to the doctor's office and pointed to where it says:

8. Fiancé(e)s.

If you are a K-1 fiancé(e) or K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiancé(e) visa, you only need to submit a vaccination supplement, not the entire medical report. You may include the vaccination supplement with your adjustment-of-status application.

The doctor quickly dropped his fee to $30

You can also do this with a copy of form I-693

2. What if I am a K nonimmigrant visa holder and already had a medical exam overseas?

If you were admitted as a:

A. K-1 fiancé(e) or a K-2 child of a K-1 fiancé(e), or as a

B. K-3 spouse of a U.S. citizen or a K-4 child of a K-3 spouse of a U.S. citizen, and

C. You received a medical examination prior to admission, then-

a. You are not required to have another medical examination as long as your Form I-485, Application to Register Permanent Residence or Adjust Status, is filed within one year of your overseas medical examination.

b. You will, however, be required to complete Part 1, Information about you, and submit the vaccination section of Part 2 with your adjustment of status application. A designated civil surgeon must complete the vaccination section and Part 5, the Civil Surgeon's Certification.

They may insist on TB test, this is expected and required.

You need to just be armed with USCIS directions and be firm with them.

I'm from Canada and I just sent in the Vaccination Worksheet that I was given at my K-1 medical exam. I was approved with no problems.

For my daughter's AOS (she was a K-2), I got the civil surgeon to transfer the information from the Vaccination Worksheet to the Vaccination Supplement. The civil surgeon charged us $35.00 to do this.

We had to call around to several civil surgeons to get that price. A lot of others wanted much more money, upwards to $200.00 for the same service.

You use name that you want on EAD and/or Green-card, if that is married name then use married name.

The forms have places for you to put maiden name, (Other Names Used) and on I-485 (Name on I-94)
