I received my packet three today. There was only ONE DS 230 part 1 and Part 2 and a checklist.

SHouldn't there have been two of these for my daughter and I.

Also what about 156 and the 156K and the I-134 and I -864 ? these didn't come

and no info about the medical .

Please help me I'm going through Montreal

I am about to lose my mind , I just so confused........

You can download all the forms on the internet, that shouldn't be a problem as long as you know which ones you need to have for Montreal consulate. As far as medical, I am sure I've seen a listing of authorized doctors somewhere on this site. I can send you name and phone of a dr. if you let me know in which city in Canada you live.

I wouldn't rush to the internet and download the forms just yet. Montreal has been known in the past to get a little snarly with people who use downloaded forms instead of the ones provided in the packets. That is why I always advocate using the downloaded forms for practise. I personally would get on the phone and/or email the consulate and check to see you have all the documents required for you and your child.
