I think on the G-325A biographical data form under AOS in sample forms.............it says first name and the

last name is entered as "Smith"

when it is suppose to be the first name of your husband or wife.....check it out.

Am I correct here?

I see SMITH as "family name", Jane as first name, Joan as middle... is there anything strange?

Ah, finally I got it

When I prepared the form, I thought that the husband name was Smith Smith It's an example, any name would be possible. Well, it's written in capital letter, so now I think you guys are correct.
