Timeline

My fiancé and her son are schedule for their embassy interview on July 21. Her son attends university and has a complete vaccination record from his school.

Is it necessary to have this record translated to English in order to use it for the AOS? If so where can we get this medical record translated--it is written in longhand and it seems like it will be difficult for anyone to read it if they are not familiar with medical terminology.

I have read that it is important to take x-rays with you to the US to be used for AOS. When are we given the X-rays and can we keep them?

Take the vaccination record with you to the medical and have them transcribe to the DS-3025. You can then use the DS-3025 for the basis of your vaccinations for AOS

X-ray is NOT needed for AOS but you will receive it after the medical.

As far as I know, the approved medical clinic(s) are the only facilities with authorization to fill out and sign the Department of State (DS) forms.
