I-693 A where do you get this?

hi everyone...checked the immigration forms on the uscis website and i didnt see the I-693A ...does this mean the one that is here in VJ is IT? or is it the vaccination page on the I-693?

The civil surgeons have form I-693A. They will fill it out for you. You can take your Vaccination Worksheet from your medical exam for your visa to a civil surgeon and they can transfer the information onto the Vaccination Supplement (I-693A) for a fee. Check with a few civil surgeons first because their fees vary greatly. We paid $35.00 for the civil surgeon to fill out the I-693A.

As of May 1, 2008, there is no longer a form I-693a. They discontinued it. You know need to fill out I-693. There is a separate vaccination section on that form itself.

the I-693a is a supplementary form of your vaccination. if you will go to the office of any uscis designated surgeon, they will transfer your foreign vaccination record to this aforementioned form. BUT, I read on some post here that started May 2008, the I-693a is no longer acceptable instead you just need the I-693 form.

I-693a no longer exists; only I-693. If your medical was conducted less than a year ago, you fill in part 1, the civil surgeon fills in the vaccination portion of part 2 and all of part 5. Instructions and form are here:

GOOD ANSWER. I actually took the instructions with me to the civil surgeon and read it to him. Since I am a K1 I read this part to him:

SECTION III Frequently Asked Questions, Question #2 (belongs to the instructions to I-693)
