Missing A# in completed I-693

Hi all,

I have a question as regards my I-693 (Medical Examination and Vaccination Record), which is part of my application to adjust status (I am a K-1).

Since I'd done my medical exam at the end of March of 2008 at my home embassy, I figured that I only needed to have my vaccination records (DS-3025) transcribed by a civil surgeon for the purposes of this application. Indeed, I used the well-known Dr. Arnold that many people on these forums have suggested is fast and efficient and indeed I received the "sealed envelope" a few days later :star: ...

... only to realize that I'd left the A-number blank in the bio info (Part 1 - Info about you)! :crying: All my other info is filled in properly in that section (including my SSN number) except for the A-number box which was left blank.

So do I need to repeat the process again (go to a civil surgeon etc) to have my I-693 filled in again or is it OK to attach the sealed envelope with my application process as is and simply e.g. perhaps add a written and signed statement that I left out the A-number there by omission or something? Any suggestions on what the best course of action would be, would be greatly appreciated!

Hello,

I forgot to add my A number on the I-693 also, they never ask me for a RFE. And i had my finger appointment in the end of june.

Don't worry, they have copies of your NOA2 on the AOS packet and they can find your A number.

No, do not worry, send it anyway with AOS application you will be fine, doubtful if you get a RFE over it. We sent ours in and nothing happened it was just like yours from Dr Arnold also. :thumbs:

Nice one mate, thank you for the reply! Both you and Miduvil are :star:s! :)

What I actually did after all, was simply write my A-# on top of Dr. Arnold's sealed letter (on the side which has my name and date when the form was filled of course, not the other one which has Dr. Arnold's stamp!). And as Miduvil pointed out, the A-# can be found elsewhere in the application set; almost every other form asks for it.

Again, cheers for your help, guys! One less thing to worry about :D

P.S. On a side note here, speaking of Dr. Arnold; some of the info on him and the I-694 process do need to be updated, I think; I promise I will be updating information on this part of the process with useful tips as soon as I find some time after finishing filing; others need to benefit from our exp like we benefit from theirs! :)

Wow, we did the same thing, no alien # or ss# put on the form. We did not do this because Dr. Arnolds office told us to absolutely fill in nothing in part 1 accept for My husbands signature.

So hopefully we all will be fine. No RFE's. :)
