I-134 Affidavit of Support

Hi all,

I am posting hoping that those of you that have completed the I-134 can help Dave and I out. David is coming to Ontario this week so he is getting all his info needed following the receipt of the packet 3 to bring to me. One of which is the I-134.

Two questions.

Question three on page 1.....

asks who the affidavit is being executed on behalf of.....obviously Dave will put in my info there. Below that also part of that question it asks for the spouse and children that may also be accompaning me. For spouse am I correct he would put "none" and then put in my two daughters info?

Question eleven on page 2...

question regarding contributions of support of those listed in #3. You have to tick that you intend or do not intend to make specific contributions to the support of the persons listed in #3 (as discussed above)...if you select intend then you have to give the exact nature and duration of those contributions...ie monthly, weekly, room and board etc....how do we answer this?

Great thank you for the response echomyst. I looked over the example forms and I guess looking at it that way makes it much more clear. Duh on my part. Glad that I questioned the "intend or do not intend" question for sure. I was baffled on that and I don't think I would have left them both unselected with an explanation as the example form shows.

Another question regarding the I-134. On the second page it asks about stocks and bonds and the market value of such. Where would you include 401K's? Should they be included in savings and banks or under stocks and bonds?

Also, anything that doesn't apply, meaning there is no answer for etc....should we have N/A or none, or just leave them blank?
