how to get NBI clearnce what i will be using MY married name or my single lastname

i will be getting a NBI clearance now i dnt know what to put in,, MY MARRIED SURNAME or MY SINGLE SURNAME...

alsodo i need to use my MARRIED NAME in Police clearnce Too,, PLS IM CONFUSED.. :help::help::help::help::help::help::help::help::help:

I had to renew my clearance in my married name so I could renew my passport in my married name as well. As far as I can remember, I had to go to the filing center at the Carriedo Plaza. It was a pain instead of just going to a kiosk. I just filled out the NBI clearance app form with my married name, showed or gave them a copy of my marriage certificate. I also applied for my Police Clearance in my married, which was just a waste since I never needed it. It was such a waste of money. Plus I don't think they really do check your name in their system. They just want your money + the expedite fee (an optional thing that goes in their pockets).

Bottom line is, it is up to you on what surname you should use. Be it on your NBI clearance, police clearance, passport, visa application forms, etc etc... As for me, I just renewed/applied everything using my husband's surname to prevent any confusion in the future.
