Does the I-864 form have to be notorized? Do we need supporting evidence such as the letter from our employer, tax transcripts etc.?

No notarization but do follow the instructions that come with the form regarding the supporting evidence. Official forms that require notarization will have a place for the notary to sign.

No, the I-864 does not need to be notarized. Yes, you need supporting evidence, everything you need is listed in the instructions.
