What name to put on I485 and other AOS docs

Timeline

Hi just wondering if you come on a K1 visa you enter with your birth name but will be married. So if married and then filing should you put your birth name (because all the documents you had when you entered have this name) or should you put your new married name?

Waiting on har copy of <!--WORD2URL-01--><!--END WORD2URL-01-->NVC<!--WORD2URL-02--><!--END WORD2URL-02--> approval and receipt of packet at consulate

Called State Dept. after not hearing anything since Sept. 23: Packet reached the consulate and was kicked back for a secondary security clearance probably do to her type of employment - Waiting for clearance and for superpacket to be sent.

Timeline

You put on those forms the name by which you wish to be known by. I wanted to be known by my married name, so I filled it out as such. There is a place on the form that says something to the effect of "previously known by" --this is where you would write down your maiden or birth name.

You can put your new name if you wish. I fill out all the forms with my married name and then we sent a certified copy of our marriage certificate in our AOS & EAD package.

I used my married name and put my maiden name in column of "Give your name exactly as it appears on your Arrival/Departure Record (Form I-94). Also included a certified copy of our marriage certificate.

April 4, 2007: Infopass & it doesn't help at all!!!
April 18, 2007 :Contacting our Congressman.
May 18, 2007 : E-mail rcvd, Welcome Notice Sent! Finally!!!
April 22, 2007 : E-mail rcvd, GC has been ordered
April 24, 2007 : Welcome Notice Rvcd. Yeeehaaa!!!!
June 1, 2007: GC arrived! Yippy! USCIS Free for 2 years!
2008: Moved to China
2009: Conditional GC Expired and Husband has been having affairs since 2008 in China. Can't file for ROC since he got laid off. He came to Jakarta to live with me and my parents. He got a new job. Life resume to normal or so I thought.
March 2010: Officially separated after yet more affairs exposed just a day after my birthday!

February 2011: Officially divorced.
June 18, 2011: He married the girl he had the last affair with.

August 2014: I am engaged with my real soulmate. Not an American.
