Timeline

We contacted Dr. Arnold's office about having my husbands vaccinations transcribed. The secretary was very informative. I also asked her if she had heard anything regarding the tb test etc. now being required. She said that they have not been informed of any new changes.

Yesterday I was getting together all the paper work to send off for my husband to their office and realized that we may not have his DS 3025 form. All we were given was a form with all of the vaccinations that he received. At the top of the sheet it says... VACCINATION DOCUMEMTATION MEDICAL WORKSHEET TWO

Then it has his name, exam date, DOB, passport number, alien case number...Then the list of vaccinaations received. Then at the bottom the doctors signature.

Nowhere on the form does it say DS 3025.

Is this the DS 3025 worksheet?

He had his medical done in Canada.

My DS 3025 had the name of the form on the bottom. I had to hunt for it! Sounds like you've got the right form though :)

17th May 08 - Got married!!!!! :)

18th June 08 - Mailed AOS/EAD/AP to Chicago lockbox

3rd October 08 - Green card in hand!!

26th August 10 - Sent I-751 to VSC

31st August 10 - NOA1 from VSC

10th January 11 - I-751 approved!

14th January 11 - 10-year green card in hand!!

22nd April 23 - N400 submitted online; NOA available in USCIS account immediately
6th November 23 - Interview; approval same day
28th November 23 - Oath ceremony scheduled

My DS 3025 had the name of the form on the bottom. I had to hunt for it! Sounds like you've got the right form though :)

Hi Everyone

We have a copy of the DS-3025.

Does this NEED to be trasferred to the I-693A or can we go to the interview with the DS-3025
