NBI Clearance

I got my NBI clearance. but i made a stupid mistake. it just like this. i was just renewing my NBI clearance but changing my last name to married name. the form just goes like this.

fatther Surename husband Surename Given Name Middle Name

so i ask one of the applicant on what i am going to put for my middle name since they already ask the fahter suremane.( my father's suremane is my middle name now since i am already married) so she told me my mom's maiden name. but i don't know what come into me but instead of putting my mom's last name(maiden name) i put her middle name. do you think they will question about this? pls help!

Just to be on the safe side rectify it asap, they might ask you later about the error on your NBI clearance.
