Evidence of Bonified relationship for lifting conditions

Does anybody have a form letter of what to write to the USCIS as an affidavit stating that we are a bonified couple? We have bank accounts together, phone is on a family plan, but for now we don't live together (long legitimate story). Anyways would anybody have any ideas on what to state? Should it be simple and sweet or does it have to be drawn out into a novel?

Bonafide,

it is bonafide... not "bonified"

It should be short, state names, addresses and places of birth of the people writing it and it should describe briefly how they know of your relationship and state that your relationship is good. Date and sign.

March'07 NOA1 date, case transferred to CSC

June'07 NOA2 per USCIS website!

Waiver I-751 timeline

July'09 Check cashed.

Jan'10 10 year GC received.
