Hello everyone,

i just got the I-693 in the mail from Dr. Arnold in Texas. I checked the copy they gave me and found out that they checked DTaP instead of Tdap. In my DS-3025 and immunization record, it is Tdap. I read that online that Dtap is for children while Tdap is for adults. It's the weekend and i guess the Dr's office is closed. I will call them on monday. In the mean time, has anyone had here had their I-693 transcribed by Dr. Arnold? is it DTap or Tdap that they checked on the form?

Hello everyone,

I didn't go through her but I remember the CS wrote down my Td shots different from my vaccination records. Just don't worry about it and submit the sealed envelope. I'm sure Dr. Arnold knows what she's doing and have done the paperwork hundreds of times.

I called Dr. Arnold's office..and yes..indeed they made a mistake..tsk tsk tsk. They said i need to mail them back the form so that they can correct it. Bad, bad, bad.
