Timeline

Hello Everyone,

Just wondering if you could give me the exact address where CFO seminar is conducted. Also, their contact number. Is it by appointment or can I just go to their office right away and attend? Also, July 10 is the date of my flight. So am wondering how much is the travel tax or are there any other fees to be paid at the airport?

Timeline

I recently had my CFO guidance and counseling last June 19th. We went to Citigold center in Quirino Ave but we were told that they no longer conduct counseling there. we were redirected to PRSIM at Taft Ave.

PRISM

2nd Flr., Banco de Oro Bldg.

2422 Taft Ave. ( a few blocks opposite of Dela Salle Univ.)

I got my yellow certificate of attendance in PRISM with a fee of 250 php

Next, I went to:

Citigold Center

1345 Pres. Quirino Ave. cor. South superhighway

Tel no. 561-8321 loc. 104 to 105

paid 400 php and got my green Guidance and Counseling certificate and will return once I have my visa for the sticker.

The requirements that I submitted for the guidance and counseling at PRISM were:

1. photocopy of my valid PRC license

2. photocopy of my valid NBI clearance

I was asked about my birth certificate and I told the lady there that I don't have it in NSO secpa since I am still waiting for the court's decision on the petitioned I made for correction of entries.

Any 2 valid IDs will do actually. Just submit the photocopies. My lady counselor was very nice, the counseling was a breeze actually. We waited for 10 mins for our certificates. Be sure to check the spelling of your name and have it corrected if it was spelled wrong right there and then. They will correct it. Photocopy the yellow certificate before going to citigold for the green cert and sticker.

They have already given you great answer on CFO...

about the travel tax. If you bought your ticket in any local travel agency here in the Philippines, the travel tax is usually included already. But if you purchase it outside the Philippines, you would need to pay travel tax: P1,620 if you will be travelling economy class and then P2,700 if you would be travelling first class.
