My husband's name is in the following format in his passport:

Carlos Marcelo Orizio de Vasco (Orizio as his mom's last name and "de Vasco" as his dad's last name)

On a previous US visa it is listed as:

Carlos Marcelo Orizio de (as first + middle)

Vasco (as last name)

although "de Vasco" is considered his last name here in Brazil.

Should we fill out the forms with "de Vasco" as his last name and "Marcelo Orizio" as his middle name? Or conform to previous US visas and put "Marcelo Orizio de" as his middle name?

p.s. sorry for the double post, but I wasn't sure which forum to put it in and no one has offered any help so far...

Use his correct name in his passport. You can use the name they put on his visa as other names known as.
