Have a few more questions about the G-325A and I-130...

1) My husband's name is in the following format:

Carlos Bob Smith de Vasco (Smith as his mom's last name and "de Vasco" as his dad's last name)

On a previous US visa it is listed as:

Carlos Bob Smith de (as first + middle)

Vasco (as last name)

although "de Vasco" is considered his entire last name.

Should we fill out the forms with "de Vasco" as his last name and "Bob Smith" as his middle name? Or conform to previous US visas and put "Bob Smith de" as his middle name?

2) Several forms only ask for city and country. Is it helpful to put the state as well (as in the case of Springfield, where there is a Springfield in several different states) or should we dumb it down and just put the city and country?

3) for parents names do we literally just put First and Last (omitting middle names and secondary last names as in the case of Latin American names)? His mom has something like 6 different names in her whole name...

23 June 2008 - Filed I-130 at US Consulate Sao Paulo, Brazil

8 August 2008 - Instructions for DS-230 received from embassy (Combined NOA2/ Packet 3)

20 October 2008 - DS-230 sent to US Consulate

23 October 2008 - DS-230 received by US Consulate Rio de Janeiro

17 December 2008 - Interview!!
