I 6931 question...

Where do we get the I 693a form? USICS website?

We are going to have Dr. arnold in texas do the vaccination supplement form. Do we just mail the I 693 a form and the DS-3025 and of course the #20.00?

Is this all we need to do?

Many have been successful (my wife included) just sending the DS-3025 with the I-485 application. If it were me I would save the $20.

that's all we did.. they told my husband to sign page 1 of the I693, and then send also ds3025 and a money order(that's what we did)..they are very thorough and fast... they even told him that he was missing a shot.. so i took him to get a shot, sent the letter showing that he had the shot and then they added that one to the information i previously sent.. they are wonderful if you ask me..

There is no 693A anymore, USCIS as updated the I-693 with directions for K-Visas, it has a vaccinations section and directions to do just that section for K-Visas.

The special instructions are for K visa applicants who are submitting the I-485 after one year of admission to the USA....

NOTE: If you are applying for adjustment of status at least one year after your first admission to the United States as a refugee or as a "K" or "V" nonimmigrant visa holder, see Questions 1-3 in Section III, Frequently Asked Questions, before proceeding any further.

I would simply submit the DS-3025 as is... If you want to transcribe, the Civil Surgeon has the forms you do not need to worry about finding them.

Canadians are required to complete their vaccinations as part of the medical for visa approval. He does not require an I-693a because the vaccination information is part of his packet handed in at the POE which will be consolidated with his AOS application once that's submitted. If he was given a copy of the DS-3025 vaccination worksheet he can submit that. If you insist on filing an I-693a, the civil surgeons have that form. You do not need to supply it to them.
