Hi all,

I'm helping my fiancee fill out the embassy forms this week and I noticed they said:

If your children will apply with you, one copy of form DS-156 for each child must be completed in full

She has a 10 year old son who doesn't have a passport. It looks like the form will contain a lot of "N/A" and "None". Also, on question #40, should she list herself as the person preparing the document?

As usual, your advice is greatly appreciated.

Rick

Just put N/A for any questions that do not pertain to her son.

And yes, she will put her name as the person preparing the form and she can sign it on his behalf.

According to the embassy instructions:

Children may be included in the parent's passport, but if over the age of 16, the must have their photograph attached to the passport

Rick,

If Indonesia will issue passports to 10 year-olds, it may make things easier all around if he has his own.

Be careful to understand what 'N/A' really means - if you say to yourself, 'don't have', the proper answer is 'none' not 'N/A', even if the reason he 'don't have' is that he's only 10 years old. You should assume that every question is applicable - if it wasn't, it wouldn't be on the form in the first place.
