So,

Here's my father in law, laying down on the hospital bed in the middle of surgery, knocked out with the preliminary stages of surgery started in his operation to remove an ulcer. About an hour into the surgery the doctor comes out to my mother in law and informs her that they do not have the specific scalpel they need to perform this surgery. In order to complete the surgery, he informs her that she must buy the scalpel for a price of $500. She agrees of course and in about a half hour in arrives the scalpel vendor to sell her the knife and they continue with the surgery.

Bare in mind that my father-in-law is a ranking member of the communist party and is supposed to be entitled to free health care for life.

Not sure if she gets to keep the knife.

I have experience in the sale of medical equipment, including scalpels. All I can say is "#######"! Is it made of gold by any chance?
