different address at the time of receiving first green card

hi, we're about to send our package, we've a little question about the address, our address was different when we got the first green card (2 years)we did inform uscis of change of address, but in the i751 petiton says on second page,question number 9: have you resided at any other address since you became a permanenet resident? if yes,attach a list of all addresses and dates)

ok, our answer to this question number 9 is of course yes!...

but underlines with very small letters it says:

if you answered "yes" to any of the above,provide a detailed explanation ona separate sheet(s)of paper and refer to "what initial evidence is required?" to determine what criminal history documentation to include with your petition.(HERE IS WHERE WE GET LOST...what is this talking about, what criminal history, what does this mean?) and it continues ...Place your name and Alien registration Number (A)at the top of each sheet and give the number of the item that refers to your response.( this is clear for us, the phrase above is the one we don't get)

please help,we don't want to send the package and then later found we did a mistake.

Never mind about the criminal history comment; that's in regards to question #7 asking if you've ever been arrested, charged, detained etc.

We had moved since the CR so we had to do this as well.

Timeline

Did you file AR-11, and I-865 at the times that you moved? If not you had better do them now before filing I-751.

Resident aliens living in the USA are required by law to report change of address within a week of move by filing AR-11, and US citizens sponsoring an immigrant need to file I-865 within 30 days of moving.

Just attach evidence of places lived like items showing other addresses.

Is now a US Citizen immigration completed Jan 12, 2012.
