Hi all,

Quick question about the vaccination worksheet.

I have the Vacination Worksheet from the Medical that I had in New Zealand just before my interview at the consulate - when I had the interview the CO photocopied and notorized it for me. When I adjust status straight after the wedding can I send the worksheet in, or do I need to get the contents of the Vaccination worksheet onto another form?

I have read different accounts of this in the forum - some people have said that you must get the information transferred by a cival surgeon, while others have said they have just sent in the vaccination worksheet? Let me know? Has anone had an RFE for sending in just the Vaccination Worksheet?

Any help anyone can provide will be appreciated.

I sent in the Vaccination Worksheet and did not get an RFE for it.

Got approved with no problems.

For my daughter's AOS, I had a civil surgeon transfer the information from her Vaccination Worksheet onto the Vaccination Supplement. The civil surgeon only charged us $35.00 to do this, so it may be worth it for your piece of mind.

Dec. 10/04... AOS Interview - APPROVED!!!!! (Passport stamped)

Sept. 9/06... I-751 sent to NSC

May 15/07... 10-Yr. PR Card arrives in the mail

Sept. 13/07... N-400 sent to NSC

Aug. 21/08... Interview - PASSED!!!!

Sept. 2/08... Oath Ceremony

Sept. 5/08... Sent in Voter Registration Card

Sept. 9/08... SSA office to change status to "U.S. citizen"

Oct. 8/08... Applied in person for U.S. Passport

Oct. 22/08... U.S. Passport received

June 27/11..... Interview - PASSED!!!

I didn't sent anything that has to do with medical stuff. I got approved yesterday with out any RFE nor interview

But had all my vaccinations before my interview for the K-1.

And people still tell me I will get a RFE even after I am approved... yeah right, like they are gonna take my GC away from me... LOL

I think I am just going to run with it as it is - if I get an RFE I can always get it transferred then.

If your vaccination worksheet shows that you have a complete vaccination history, then I would just include a copy of the vaccination worksheet. If you needed to get additional vaccinations here in the States, then I would have the information transferred from the worksheet to the Vaccination Supplement by a Civil Surgeon. I only sent in the worksheet, but did get the supplement done to take with me to the interview - then was approved without an interview.
