Hi all:

1)On Part I of Form I 485, the current USCIS status is K1 visa Holder; and the next line indicates: Expires on: mm/dd/yyyy, do you guys put date expires on the visa or date expires from date admitted into the U.S?

2)K1 application for AoS should be sent to USCIS Lockbox: USCIS PO Box 805877 Chicago, IL 60680-4120. Is it correct?

3)Can I mail I485 in the same envelope with I765 (work permit) to the same PO Box address since there's no fee for I765 if filed together w/ I485? Also do you have to submit required documents for each application separately? ie: passport photos, I-94, copy of passport, ect?

4)Question 16 on form I765: what do you put if one enters the U.S as K1 visa?

Any help will be greatly appreciated :)

Hi kjw:

Thanks for ur response. Also did you submit a vaccination supplement -- I-693A signed by a civil surgeon in the U.S? If so how much did it cost?

For Question 1, I think it's the expiration date of the 90 days from date admitted to the U.S as I looked at the example forms from this web site, but I'm not sure. The instructions are not clear.

1) Expiration date on the I-94 card. The date on visa is only the validity of the visa, it is the date that the visa needs to be used by.

2) You file I-485 to Chicago PO box.

3) Yes you file them together to Chicago.

4) The I-765 is coded (c )(9 ) based on the I-485, NOT based on K-1

We had the supplement done for $30.

Many, just do it by mail using this one in TX. Last I heard he will do it for $20

Note: I-693 has changed some.

I-693 instructions page 4

2. What if I am a K nonimmigrant visa holder and already had a medical exam overseas?

If you were admitted as a:

A. K-1 fiancé(e) or a K-2 child of a K-1 fiancé(e), or as a

B. K-3 spouse of a U.S. citizen or a K-4 child of a K-3 spouse of a U.S. citizen, and

C. You received a medical examination prior to admission, then-

a. You are not required to have another medical examination as long as your Form I-485, Application to Register Permanent Residence or Adjust Status, is filed within one year of your

overseas medical examination.

b. You will, however, be required to complete Part 1, Information about you, and submit the

vaccination section of Part 2 with your adjustment of status application. A designated

civil surgeon must complete the vaccination section and Part 5, the Civil Surgeon's Certification.

http://www.uscis.gov/files/form/I-693instr.pdf

Again this in not a full medical, only the "supplement" is now part 2 of the I-693 form.

You should be able to print out the directions for form I-693 and point out where a full medical is not needed for K-Visa holders to the doctors that are insisting on gouging you.

Also The date to go by is when I-94 expires, 90 days after entry.

OUR TIME LINE Please do a timeline it helps us all, thanks.

The instructions are not clear.

as what the dr. Arnold requested:

I sent my vaccination record from overseas and td shot to dr. arnold. and the supporting documents are:

Copy of vaccination record from overseas

Copy of TD shot record from USA

Copy of passport with biographic information

copy of K1 visa

Printed out of I-693 Part I only

Check $20.00 for payment

We called Dr. Arnold first and leaved a message about my situation. That i only need a I-693 and transcribe all my vaccination record. Den tomorow later they called us back and let us know about the documents that need to send it to Her. So thats all.

I did sent the sealed envelop transcribe by CS with my vaccination record in it together with my AOS package. But i did not sent my vaccination record from overseas. coz its in the sealed envelop already the dr. put all together.

hope this help....
