Timeline

Hi everyone. We are now in the process of preparing our documents for aos. We are wondering if anyone can recommend a civil surgeon in the bay area (preferably fremont) who is willing to transfer the vaccination records to form i-693a without requiring a new medical exam. The ones we talked to so far would not do it unless I undergo a full medical exam.

We appreciate any help on this. Thanks.

Many, just do it by mail using this one in TX. Last I heard he will do it for $20

2. What if I am a K nonimmigrant visa holder and already had a medical exam overseas?

If you were admitted as a:

A. K-1 fiancé(e) or a K-2 child of a K-1 fiancé(e), or as a

B. K-3 spouse of a U.S. citizen or a K-4 child of a K-3 spouse of a U.S. citizen, and

C. You received a medical examination prior to admission, then-

a. You are not required to have another medical examination as long as your Form I-485, Application to Register Permanent Residence or Adjust Status, is filed within one year of your

overseas medical examination.

b. You will, however, be required to complete Part 1, Information about you, and submit the

vaccination section of Part 2 with your adjustment of status application. A designated

civil surgeon must complete the vaccination section and Part 5, the Civil Surgeon's Certification.

Again this in not a full medical, only the "supplement" is now part 2 of the I-693 form.

You should be able to print out the directions for form I-693 and point out where a full medical is not needed for K-Visa holders to the doctors that are insisting on gouging you.

We also live in Fremont and after making a number of calls to doctors who would require a full medical checkup, i'm glad i didn't give up and dialed his number. He is heaven-sent! He knows his job. Just tell him you came on a K-1 visa and he'll gladly transfer your record to the vaccination sheet. Take note: there is a new medical form effective this May which is basically the I-693 which incorporates the vaccination sheet. The USCIS finally got rid of the vaccination supplement (I-693a) and put it all in one form. Don't worry, Dr. Amoli will download the new form in the USCIS website and just fill-up the vaccination sheet part.

He charged me $40 for the Td shot (that's the only shot you need if you already had your MMR and varicella at st. luke's) and $30 for transferring your record. Make sure you call him first and make an appointment and ask how much he would charge (you can negotiate) and when you finally come for your appointment, remind him the amount he told you on the phone.

He is really nice, might be a bit difficult for you to understand his english because of his accent (ha!ha!ha!) but that's ok. He will put the form in a sealed envelope - 1 copy for the USCIS A-file and 1 copy for you.

And tell him MRS. FIGUEROA recommended him even if he pronounced my surname as FIGARO (hihihi)

by the way, we had our AOS interview this morning in San Francisco and we were approved!

and by the way, about the Dr. in Texas, i think he has become so popular that the USCIS is starting to notice it :D

She raised eyebrows when she said that you live in California and you had a medical in Austin? After explaining my reason why I did what I did, she asked us to sign and date a sworn statement that said "my medical was completed, but never did I meet this doctor, go to office or examined by this doctor, and that business was conducted by mail". She said that we are approved but, we might get a call from State and Federal officials regarding the practice of doing mail vaccination supplements."

Finally somebody pointed this out!!

I was wondering that too, I live in NY and how come my medical in TX??? Since I got a RFE for the medical, I didn't want to make any more mistake, so I found a local civil surgeon in my area at last. Checked usps just now, I sent the RFE back on May 28th and it was delivered on May 30, 2008, 11:22 am, LAGUNA NIGUEL, CA 92607!!! Yeah!! Just hope to finish the AOS processing asap. :)
