vaccination document

hey everyone,

when i took the medical exam, the doctors told me that when i get here i need to have another batch of shots that were given back in the philippines and one other thats only available in the us.now, i checked my papers and somehow the document`s not there.im wondering if i still need it for validation or record that i indeed got those shots.if so, how do i get another copy from the clinic?hope to hear from anyone here.thanks

You can call to the clinic to get a copy of your vaccination's record. When you come to US you need to have the I693A form filled by a surgeon authorized by immigration. You can go to the doctor here and ask for an order to get the vaccinations in a public hospital for free. The vaccinations are MMR, rubella, chicken pox, tetanus and diphteria.

Call St. Luke's and ask for the copy of your vaccination records, or else you have to take all those shots again here in the US.
