Local Civil Surgeon refuses to provide I693A

The instructions on the I 485 say what K1ers need for AOS (vaccination supplement).

If he won't relent, use the doctor in the second post on this page.

Hello all. I am going today to my doctor's appointment to get the I-693A filled up. Unfortunately, the doctor that filled in all my papers back in Greece, never gave me a copy, so I really hope the fax with the list of vaccines he sent me is going to work.

When I called the local doctor, his receptionist told me that I would have to pay $250 for a whole medical examination, but after I explained to her that I just need the vaccines, she seems to understand. We'll see later I guess.

A question:

is this the form I need to print out and give to the doctor?

http://www.cdc.gov/ncidod/dq/pdf/ti-03/appdx-a_693vacc.pdf

and is there a form online that I could print out

to convince the doctor that I don't need the full med exam?

Definitely bring the instructions for the I-485 with you. It says right on it that K1 applicants don't need a full medical if done within a year.

Also check your PMs, I just sent you one.

well, I've just phoned about 15 places, and the only one that would do just the supplement wanted $125.

A call to Texas is in my near future, I think.

This is very common, and yes, I would like to report the doctor as well who almost kicked me out of his office because I asked for an explanation why he refused to even read the instructions from USCIS that state cleary who does and does not need a full medical..LOL

He wanted $260 for a full medical and he told me straight in my face that he was the only authorized doctor here to do it, so I better comply if I wanted my greencard.

So I went to a doctor 180 miles away and he filled it in for $30! Long drive but I got to go on a shopping spree in Dallas , so it wasn't bad at all..LOL

Ay Naku!, Hay Caramba!, Oh My Goodness...

Only in the Government would you see this: See below for govern_MENTAL logic!!

APPENDIX A - SUPPLEMENTAL FORM To I-693

3. Results (Mark only 1 which applies)

_ Applicant may be eligible for blanket waiver(s) as indicated above.

_ Applicant will request an individual waiver based o n religious or m oral convictions.

_ Vaccine history complete for each vaccine, all requirements met.

_ Applicant does not meet immunization requirements.

If everything with your Vaccination situation is "All Set" and "Good to Go" for your AOS process, which Item in Section 3, Results do you think would make sense?

.

.

.

.

NOPE!!!! We're Wrong!

Take a look at the USCIS instructions to the Civil Surgeon for filling out the form....

• Vaccine history complete for each vaccine.

If the applicant has met vaccination requirements for each vaccine recommended, the “Vaccine history complete for each vaccine, all requirements met” box must be checked; however, this will probably not occur.

Vaccination history incomplete - Applicant may be eligible for blanket

waivers(s).

<snip> This (option) will probably always be checked because some vaccines may not be age appropriate for the applicant <snip>

Sooooo...now explain to me the difference between "for each vaccine recommended" and "some vaccines <are> not age appropriate.." :protest:

I don't find it that confusing. A vaccine may be recommended, but only if its age appropriate.

In other words, you can only have ALL the vaccines that are recommended if you happen to be of an age where they are all appropriate - which is probably unlikely to happen.

I waited an hour today at a doctor's office in Laredo, TX and paid $75. The Civil Surgeon said it's the first K-1 he's ever done, but I prepared everything he needed filled and highlighted the instructions on the I-693 instruction form:

http://www.uscis.gov/files/form/I-693instr.pdf

For the K-1 Visa, the only parts that need filled are Part 1, Part 2 (vaccinations only), and Part 5.

He gave it to me in a sealed envelope. He filled "not done" in the empty parts, but I don't think it will be a problem. I'm sure USCIS gets strange paperwork from the civil surgeons all the time. Does anybody think that will be a problem?
