We are having problems getting my wife's vaccination worksheets transcribed to the I-693A. My wife has no records showing the dates for the shots she has already had. Her vaccination worksheet only has a check mark in the column showing the series complete. The Civil Surgeon we went to said they could not transcribe them without the dates. They also had no answers for us on what to do. Another problems is we're running out of time to file for AOS. We must file before June 2. If we had to, could we just send in the worksheets(as I have read that some have done this)? What happen if we get a RFE?

I just read this in a post from captain ewok.

"If the beneficiary's vaccinations were COMPLETED as part of the visa process, IT IS NOT NECESSARY TO INCLUDE FORM I-693A IN THE AOS SUBMISSION. All your medical information is enclosed in the brown envelope which was handed in at the POE. The envelope is then forwarded on to USCIS where it will be consolidated with the I-485 submission, once received. If you received a copy of the Vaccination Documentation Worksheet (Form DS-3025) from your Panel Physician, you may include a copy of that form in your AOS submission in place of Form I-693a."

As I mentioned in my first post, on the vaccination worksheet, the vaccinations are checked off as "completed series". Does this mean I do not need the dates and I do not have to submit the I-693A?

Yes, as Captain Ewok says, if it is completed, you could send DS-3025 instead of I-693A. It's your choice. Some received RFE, some not.

Usually, the worksheet has date for each record... weird :blink:

If you don't have time, maybe worth to try with DS-3025, and in parallel, you could contact panel physician who wrote DS-3025 in her country and have the doctor add dates.... prepare for RFE just in case. (Basically your fiancee provided dates to panel physician, right? )

It is really up to you. Most people will tell you that you MUST have the info transfered to the I-693A or you will be denied. I myself are in the process of the AOS and didn't submit the I-693A or any medical info at all. So far I didn't get a RFE ;) I can only tell you that many people didn't sent the I-693A and were approved without a problem. Other have had a RFE...

If there is an easy and inexpensive way to get the vaccination info transfered to the I-693A you should do it! But since you already mentioned "The Civil Surgeon we went to said they could not transcribe them without the dates." I would do like many others did before you and me, and not send it at all. What I would do though is on the cover letter explain that you completed everything and that the info was in the envelope that you handed over at the POE.

NO, She has no records for any of the vaccinations she has ever had. The physician in Peru just checked everything off. This did seem a little wierd to me at the time.

Wow! :blink: Did the physician test her blood for antibody? If she has all the appropriate antibody recently tested, the date is not that important, I guess...

Just as a side note, when adjusting status from a K1 visa, there is no time limit on the filing for AoS. Filing sooner means you can get your EAD sooner of course, but you don't have to send your package off within a set number of days, you just have to have been married within 90 days of entering.
