We are preparing the forms for the AOS, and i would like to know what immunizations is necessary.

I had the DS-3025, it's written

results: vaccine history incomplete

Here is more information - it is from the directions to panel physicians advising them what to look for and what immunizations are required - there is a table after the text that shows the required immunizations at which age:

I have one another question

I'm a little worried about the varicella; is it a requirement ? i have no vaccination.

And i have 38 years old.

Did you read the document Kathryn41 provided above?

According to the doc, yes, it is requirement unless you already have antibody.

Also, you can tell which one you should take from DS-3025...

If "Not age appropriate" is checked, you don't need them. Items "Insufficient time interval" or "Not routinely available" are to be completed. Though I'm not sure about "contraindicated"... I believe you have to talk with medical doctor.
