Here is the list of various vaccinations required. Some are age specific - it is not necessarily a one size fits all:

Here is more information - it is from the directions to panel physicians advising them what to look for and what immunizations are required - there is a table after the text that shows the required immunizations at which age:

I have one another question

I'm a little worried about the varicella; is it a requirement ? i have no vaccination.

And i have 38 years old.

Did you read the document Kathryn41 provided above? :unsure:

According to the doc, yes, it is requirement unless you already have antibody.

Also, you can tell which one you should take from DS-3025...

If "Not age appropriate" is checked, you don't need them. Items "Insufficient time interval" or "Not routinely available" are to be completed. Though I'm not sure about "contraindicated"... I believe you have to talk with medical doctor.
