DS-156 question

On question 18 of the DS-156 my fiancee and I are not sure what to put in there. She has been married before and is divorced and it says even if divorced or separated, include maiden name. Am I to assume she puts HER maiden name in there or should we leave it blank? Same with number 19. Since there is no "spouse", leave it blank?

She will put her former husband's name there. Since her husband wont have a "Maiden Name" maiden name does not apply.

Note: Maiden name there would apply if the person filling out the DS-156 were male, and was listing EX-Wife in question 18, and indicate the EX-Wife's maiden name.

For 19 this is EX-Husband's DOB

Thanks. Don't understand the logic behind that one, but then again I don't understand a lot of what US immigration does.
