Are you the petitioner or the one being petitioned for? If you are the petitioner then the following apply.

1. Recommended is the last 3 years.

2. Federal tax returns are usually Notarized.

3. No, all financial information must come from the USC.

4. No

5. The company refuses to provide a letter stating such a simple fact? Are you sure about that? Sounds like a company with hard people do deal with in that case.

6.Everything looks fine, however I would include pay stubs. Three recent pay stubs would be helpful.

7. Make at least 2 copies of each document. You never know what could happen in the future. A copy can always be a life saver in any odd circumstance you might run into.

I disagree with BigCityDave - tax returns are not needed to be notarized - I have sent copies from turbo tax throughout the process as have others without any notarization; It doesn't matter if the employment letter doesn't say permanent - nothing is permanent here - anyone can be fired in a day or a company can go bankrupt - none of my employment letters said 'permanent' - the most important part of the letter is the amount of money you are making (to meet the 125% requirement); pay stubs have limited value if you have the employment letter; I never submitted pay stubs.

Motu, You are a good person for this question. I just sent my I-134 and supporting evidence to Colombia with my uncle who happens to be flying down today. I will be attending the interview and will be taking additional copies. I just want to have copies already in Bogota just in case.

I printed my last three returns out of TurboTax. My only concern was about signing them. It wouldn't make sense to me to sign them as they are printed copies. Did you sign yours?

Either way, I have never heard of a need for the copies of returns to be notarized. My employment letter doesn't have the word "permanent" on mine either, but I doubt that most companies would put that as they probably don't want to potentially expose themselves legally. I did have my company put that it was specifically for my wife's K-3 with case number and everything. It seems like that couldn't hurt since it shows that my employers know about my marriage.
