Basically, documents that are essentials lang just to prove that there's a genuine relationship.

Here's the checklist of the documents that I've submitted:

Documents included:

A. Primary Documents:

• CRBA / Passport Application Special Attachment -optional (My fiance made this letter before he went back to maine last July 11, explaining why he can't attend the crba appointment and that Rohan was really conceived by me..hahaha)

• DS-11 - Application for a U.S. Passport

• FS-579 - Application for Consular Report of Birth Abroad (CRBA)

• MNL/PPT 15 - Affidavit of Parentage and Physical Presence

• DS 3053 - Statement of Consent: Issuance of a passport to a minor under age 14.

• Applicant's passport photos – two (2) 2x2 photos

• Applicant’s birth certificate issued by NSO on security paper

B. Father’s Proof of U.S. citizenship / U.S. Physical Presence

• Certified true copy of U.S. birth certificate

• Notarized Copy of Current Passport – Date of Issue: 23 May 2008 – 22 May 2018

• Original Passport – Date of Issue: 09 October 2007 – 08 October 2008

• Original Passport - Date of Issue: 11 August 1997 – 10 August 2007

C. Parent’s physical presence in the same location at the probable time of applicant’s conception:

• Notarized Copy of Visa Pages

o Philippines – 16 August 2007 to 15 October 2007

• Bureau of Immigration Receipt

o Visa Waiver – 21 August 2007

o Visa Waiver – 11 October 2007

• Lease of Contract Agreement Monte Carlo Bldg. at East Ortigas Mansions, Pasig

o Dated: 16 August 2007 – 16 September 2007

o Dated: 16 September 2007 – 16 October 2007

• Boarding Pass
