Can someone tell me how to do the parental consent? Or does anyone here has an example? please let me know, thanks ahead.

Hi Nova, thanks for having this in mind..

To my fellow Vj family,

My hubby will be flying this monday from Dallas, and we are preparing for docs for our wedding. We both needs to have parents consent. I'm 19 and he is 22..

Ate riza, from whom can I get an example of it, and does it still needs to be notarized from his state?

we kinda need to have info as soon as we can cause he only have until fri to accomplish this thing.
